As the disc spins due to momentum, molecular collisions in the centre start to become more common, and the heat starts to rise. As more and more matter began to collect in the middle the Sun started to take shape as an extremely hot and dense protostar. Over time the mass and heat reached the point where nuclear fusion was possible, and the Sun as we know it today was formed, complete with a relatively small amount of leftover material that would go on to form the planets.

The formation of planets is again down to gravity. As the dust particles began to collect to form large rocks they would grow slowly larger through collisions. These early formations are known as planetesimals. The outer solar system was the only area where it is cold enough for molecules such as methane and water to condense, and so the inner planets are formed from heavier rocky elements. These elements are relatively rare and so the inner planets could not grow as large as the gas giants that would eventually form from lighter elements further out in the Solar System.

Initially there were up to a hundred small planets orbiting the Sun. Over the next hundred million years these would collide and merge to form the four rocky planets and their moons that we have today. It is thought that the Earth’s moon was formed from a collision with a Mars sized object towards the end of this period. Around four billions years ago the Earth and other terrestrial planets would undergo what is known as the Late Heavy Bombardment. This is a period in time that lasted for several hundred million years where the inner planets where peppered with meteorites and asteroids. The high number of impacts is credited to the migration of the gas giants causing gravitational instability in the Solar System. Evidence of this is present in the highly cratered surface of the Moon and Mercury as well as several large craters on Earth.

Once the Late Heavy Bombardment was over it is believed that solar winds will have pushed much of the excess material out into interstellar space and that the Solar System settled into what we see today, with nine planets and an asteroid belt separating the gas giants from the rocky inner planets.

During and shortly after this time the Earth was very different to what it is now. Known as the Hadean eon the world was hot, dry and extremely volcanic. There were no oceans or oxygen in the atmosphere and the surface was molten. Due to this liquidity the heavier elements fell to the centre of the planet while lighter ones would remain nearer the surface. This process would eventually give rise to the layered structure we see in Earth today.

The next events on Earth would eventually give rise to the planet being hospitable for life. As gravity allowed more of an atmosphere to be retained temperatures would fall to such a level that the outer layer of the planet cooled and a rocky crust was formed. As gases were released from volcanoes and asteroid impact the Earth built up a significant atmosphere that contained water. As clouds formed and rain began to fall the oceans filled up. Within 750 million years the Earth had oceans of water and a rocky crust, although there was significantly less land then than there is now. While the atmosphere didn’t yet contain an ozone layer and very little oxygen the stage was set for the evolution of life. There was liquid water, a stable temperature and enough organic chemicals in circulation that would allow bacteria to thrive.

Where the term conflict diamonds is used is when the diamonds are traded illegally, which is believed to fund conflict and military weapons in war torn areas, such as central and Western Africa. The first known instances of conflict diamonds came about during the early 1990’s in Sierra Leone, whereby illicit mining of diamond were performed funding the conflict ridden country. Other areas of conflict were notable in Liberia, Angola, the Democratic Republic of the Congo and the Ivory Coast.

Sierra Leone was one of the countries which had suffered a brutal civil war for over ten years, from 1991 through 2002. This had sparked the Revolution United Front (RUF) who had terrorized the population, taking control of the diamond trading and using the funds to prolong the war. Approximately half a million of Sierra Leones people were forced to flee the country, with thousands missing and numerous killed.

The UN had established one of the biggest Peacekeeping group (UNAMSIL), sending a mission to Sierra Leone in order to disarm and demobilise the illegal trading. By 2002 the mission had succeeded in their goal, abolishing the conflict diamonds trade. The mission had also enabled peace in Sierra Leone, with the country becoming a Democratic country. However, the diamond mining now plays a crucial part in developing the country, with the funding enabling a more stable future for the country. This is done through Kimberly Process Certification Scheme (KPCS).

The KPCS was set up in 2003 to prevent the illegal trading of rough diamonds, by creating a non-conflict rough diamond trading. In order for countries to participate they must ensure that the diamond gems are traded legitimately and are not used to fund any rebel group of any sort. These countries will need to become a member and must go through a strict procedure to warrant them as legal trading. This has contributed to the abolition of conflict diamond trading.

In November 2006, 71 countries had become members of this organisation making it possible to stop conflict diamond from entering into the trading process. Since this process has been in place and actions have been taken to counter conflict diamond trading, countries like Namibia, Sierra Leone, Liberia and Angola have received the benefits of the diamond trade. This has enabled improvements in the healthcare system, with Sierra Leone exporting three percent of the worlds diamond in 2006.

Diamond manufacturers in the UK now trade with conflict free diamond, making sure that they provide sufficient certification to prove it is conflict free. The Kimberley Process functions on the basis of safely trading diamonds, continuing to reduce the level of illegal diamond trading to less than one percent (a dramatic decrease) and investigating into any suspicious trading that may be operating.

1. Types of Diabetes
There are four forms of the disease, classified as: types 1 and 2, gestational and other specific types.

a. Type 1 Diabetes
In people with type 1 diabetes the pancreas is not producing insulin, so blood glucose levels are higher than normal. People with this form of diabetes require daily insulin therapy to survive. This form is further split into idiopathic diabetes and immune mediated diabetes.

Type 1 accounts for around 10% to 15% of all people with diabetes. It is one of the most serious and common chronic diseases of childhood, with about half of the people with this form of diabetes developing the disease before age 18. It is also known as insulin dependent diabetes mellitus (IDDM) or juvenile-onset diabetes.

b. Type 2 Diabetes
Type 2 diabetes is marked by reduced levels of insulin (insulin deficiency) and/or the inability of the body to use insulin properly (insulin resistance). This form of the disease is most common among people aged 40 years and over and accounts for 85% to 90% of all people with diabetes. Most people with type 2 diabetes are obese.

c. Gestational Diabetes
Gestational diabetes occurs during pregnancy in about 3% to 8% of females not previously diagnosed with diabetes. It is a temporary form of diabetes and usually disappears after the baby is born. However, it is a marker of much greater risk of developing Type 2 diabetes later in life. Screening tests for gestational diabetes are usually performed around the 24th-28th week of pregnancy.

d. Other Specific Diabetes Types
This includes people who have diabetes as a result of a genetic defect, or exposure to certain drugs or chemicals.

2. Causes of Diabetes Mellitus
Diabetes is caused by resistance to, or deficient production of, the hormone insulin, which helps glucose move from the blood into the cells.

In type 1 immune mediated diabetes, pancreatic beta cell destruction results in failure to release insulin and ineffective transport of glucose. There is no known cause for idiopathic diabetes.

In type 2 diabetes, the beta cells release insulin but, receptors are insulin resistant and glucose transport is variable and inefficient. Some people may have conditions that can cause diabetes, such as Cushing’s syndrome, pancreatitis or liver disease.

Risk factors for type 2 diabetes include;

a. Obesity
b. History of Gestational diabetes
c. Lack of physical activity
d. Hypertension
e. Age 45+
f. Ethnic groupings other than anglo-saxon
g. Low HDL cholesterol levels
h. Impaired glucose tolerance
i. Family history

3. Signs and Symptoms of Diabetes
Symptoms of diabetes include; dehydration, unexplained weight loss, excessive thirst, frequent need to urinate, fatigue, lethargy, severe irritation and itching, excessive hunger, blurred vision, skin and urinary infections and vaginitis.

4. Diagnosis
Diabetes or impaired glucose tolerance may be detected on routine blood tests as part of a general health check up or investigation for other symptoms or diseases. Diabetes tests check blood glucose levels.

Fasting blood glucose levels are classified as:
Normal: less than 100 mg/dl
Prediabetes: 100 to 125 mg/dl
Diabetes: greater than 125mg/dl

Because symptoms can be mild, diabetes is often detected when a person suffers a problem that is caused by diabetes, such as a heart attack, stroke, neuropathy, poor wound healing or a foot ulcer, certain eye problems, certain fungal infections, or delivering a baby with macrosomia or hypoglycemia.

5. Diabetes Treatment
Effective treatment endeavors to normalize blood glucose and reduce complications using insulin replacement, diet and exercise. For those with very mild diabetes, diet plus weight loss plus exercise may be enough to keep blood glucose within reasonable levels.

Dietary changes should focus on the ideas of weight loss and blood sugar regulation by keeping the blood sugar levels relatively stable throughout the day, avoiding big peaks or troughs.

The principal treatment of type 1 diabetes, even from its earliest stages, is replacement of insulin combined with careful monitoring of blood glucose levels using blood testing monitors.

Essentially, you want your client to trust your opinion and counsel. You want your client to bring you into their budget and strategy meetings, months before any product is purchased.

You want to be an essential — no, not just essential–a “critical” part of their team. You don’t want to be seen as just another vendor. The question is: do your clients see you that way? If not, why not? Is it because of the things that you do? Do you act like a *vendor* or do you act like a “trusted advisor”?

Here’s how you can tell:

* Are you product neutral? (Please note — I know a lot of so-called impartial trusted advisors who are product-biased behind the scenes but not in front of the customer. That is the point: you want to be seen by your client as unbiased)

* Do you demonstrate *and* communicate your intellectual property/capital? For instance, do you publish a newsletter, whitepapers and reports, write articles, etc.? Other than your certifications, how does a potential client know what’s inside your head? Or better yet, how do they find out what’s in it for them?

* Are you more concerned about your Need-to-Sell versus. your client’s Need-to-Buy? In other words, are you pushing pre-existing products/solutions before you even diagnose or understand what challenge your client is facing?

There are more ways to know if you are behaving like a “trusted advisor” but these are just a few clues.

Take a look at how you approach you customers.

I can usually tell how much a company acts like a trusted advisor by looking at their web site. Here’s how I do it: if the site is inward-looking such as “look at us, see how many certifications we have,” or if there is too much me, me, me then I know. (”We do this, we do that, we know what’s best for you–just ask us.”)

However, if the site is client-centric with everything geared toward how a client can benefit from a consultation, then I know they are on track to being a trusted advisor.

The most common cause of neuroma in ball of the foot is the abnormal movement of the long bones behind the toes called metatarsal bones. A small nerve passes between the spaces of the metatarsals. At the base of the toes, the nerves split forming a “Y” and enter the toes. It is in this area the nerve gets pinched and swells, forming the neuroma. Burning pain, tingling, and numbness in one or two of the toes is a common symptom. Sometimes this pain can become so severe, it can bring tears to a patient’s eyes.

Removing the shoe and rubbing the ball of the foot helps to ease the pain. As the nerve swells, it can be felt as a popping sensation when walking. Pain is intermittent and is aggravated by anything that results in further pinching of the nerve.

When the neuroma is present in the space between the third and fourth toes, it is called a Morton’s Neuroma. This is the most common area for a neuroma to form. Another common area is between the second and third toes. Neuromas can occur in one or both of these areas and in one or both feet at the same time. Neuromas are very rare in the spaces between the big toe and second toe, and between the fourth and fifth toes. Neuromas have been identified in the heel area, resulting in heel pain.

A puncture wound or laceration that injures a nerve can cause a neuroma. These are called traumatic Neuromas. Neuromas can also result following a surgery that may result in the cutting of a nerve.

Diagnosis

The diagnosis of Neuromas is made by a physical exam and a thorough history of the patient’s complaint. Conditions that mimic the pain associated with Neuromas are stress fracture of the metatarsals, inflammation of the tendons in the bottom of the toes, arthritis of the joint between the metatarsal bone and the toe, or nerve compression or nerve damage further up on the foot, ankle, knee, hip, or back.

X-rays are generally taken to rule out a possible stress fracture or arthritis. Because nerve tissue is not seen on an x-ray, the x-ray will not show the neuroma. A skilled foot specialist will be able to actually feel the neuroma on his exam of the foot. Special studies such as MRI, CT Scan, and nerve conduction studies have little value in the diagnosis of a neuroma. Additionally, these studies can be very expensive and generally the results do not alter the doctor’s treatment plan.

If the doctor on his exam cannot feel the neuroma, and if the patient’s symptoms are not what is commonly seen, then nerve compression at another level should be suspected. In this instance, one area to be examined is the ankle.

Just below the ankle bone on the inside of the ankle, a large nerve passes into the foot. At this level, the nerve can become inflamed. This condition is called Tarsal Tunnel Syndrome. Generally, there is not pain at this site of the inflamed nerve at the inside of the ankle. Pain may instead be experienced in the bottom of the foot or in the toes. This can be a difficult diagnosis to make in certain circumstances. Neuromas, however, occur more commonly than Tarsal Tunnel Syndrome.

Treatment

Treatment for the neuroma consists of cortisone injections, orthotics, chemical destruction of the nerve, or surgery. Cortisone injections are generally used as an initial form of treatment. Cortisone is useful when injected around the nerve, because is can shrink the swelling of the nerve. This relieves the pressure on the nerve. Up to three cortisone injections can be given over a twelve-month period. Cortisone may provide relief for many months, but is often not a cure for the condition.

The abnormal movements of the metatarsal bones continue to aggravate the condition over a period of time.

To address the abnormal movement of the metatarsal bones, a functional foot orthotic can be used. These devices are custom-made inserts for the shoes that correct abnormal function of the foot. The combination treatment of cortisone injections and orthotics can be a very successful form of treatment. If, however, there is significant damage to the nerve, then failure to this treatment can occur.

When there is permanent nerve damage, the patient is left with three choices: live with the pain, chemical destruction of the nerve, or surgical removal of the nerve.

Definition

Morton’s neuroma occurs as the nerve passes under the ligament connecting the toe bones (metatarsals) in the forefoot.

Morton’s neuroma most frequently develops between the third and fourth toes, usually in response to irritation, trauma or excessive pressure.

The incidence of Morton’s neuroma is 8 to 10 times greater in women than in men.

Symptoms

* Normally, there are no outward signs, such as a lump, because this is not really a tumor.
* Burning pain in the ball of the foot that may radiate into the toes. The pain generally intensifies with activity or wearing shoes. Night pain is rare.
* There may also be numbness in the toes, or an unpleasant feeling in the toes.

Runners may feel pain as they push off from the starting block. High-heeled shoes, which put the foot in a similar position to the push-off, can also aggravate the condition. Tight, narrow shoes also aggravate this condition by compressing the toe bones and pinching the nerve.

Diagnosis

During the examination, your physician will feel for a palpable mass or a “click” between the bones. He or she will put pressure on the spaces between the toe bones to try to replicate the pain and look for calluses or evidence of stress fractures in the bones that might be the cause of the pain. Range of motion tests will rule out arthritis or joint inflammations. X-rays may be required to rule out a stress fracture or arthritis of the joints that join the toes to the foot.

Treatment

Initial therapies are nonsurgical and relatively simple. They can involve one or more of the following treatments:

* Changes in footwear. Avoid high heels or tight shoes, and wear wider shoes with lower heels and a soft sole. This enables the bones to spread out and may reduce pressure on the nerve, giving it time to heal.
* Orthoses. Custom shoe inserts and pads also help relieve irritation by lifting and separating the bones, reducing the pressure on the nerve.
* Injection. One or more injections of a corticosteroid medication can reduce the swelling and inflammation of the nerve, bringing some relief.

Several studies have shown that a combination of roomier, more comfortable shoes, nonsteroidal anti-inflammatory medication, custom foot orthoses and cortisone injections provide relief in over 80 percent of people with Morton’s Neuroma. If conservative treatment does not relieve your symptoms, your orthopaedic surgeon may discuss surgical treatment options with you. Surgery can resect a small portion of the nerve or release the tissue around the nerve, and generally involves a short recovery period.

Problems often develop in this area because part of the lateral plantar nerve combines with part of the medial plantar nerve here. When the two nerves combine, they are typically larger in diameter than those going to the other toes. Also, the nerve lies in subcutaneous tissue, just above the fat pad of the foot, close to an artery and vein.

Above the nerve is a structure called the deep transverse metatarsal ligament. This ligament is very strong, holds the metatarsal bones together, and creates the ceiling of the nerve compartment. With each step, the ground pushes up on the enlarged nerve and the deep transverse metatarsal ligament pushes down. This causes compression in a confined space.

The reason the nerve enlarges has not been determined. Flatfeet can cause the nerve to be pulled toward the middle (medially) more than normal, which can cause irritation and possibly enlargement of the nerve. The syndrome is more common in women than men, possibly because women wear confining shoes more often. High heels cause more weight to be transferred to the front of the foot and tight toe boxes create lateral compression. As a result, there is more force being applied in the area and the nerve compartment is squeezed on all sides. Under such conditions, even a minimal enlargement in the nerve can elicit pain.

Signs and Symptoms

The most common symptom of Morton’s neuroma is localized pain in the interspace between the third and fourth toes. It can be sharp or dull, and is worsened by wearing shoes and by walking. Pain usually is less severe when the foot is not bearing weight.

Diagnosis

To diagnose Morton’s neuroma the podiatrist commonly palpates the area to elicit pain, squeezing the toes from the side. Next he or she may try to feel the neuroma by pressing a thumb into the third interspace. The podiatrist maneuvers the instruments carefully through many structures and cuts the deep transverse metatarsal ligament, which typically causes most of the nerve compression. This procedure can lead to instability in the forefoot that may require attention in the future.

The second procedure involves a plantar approach, in which the incision is made on the sole of the foot. The patient must use crutches for about 3 weeks and the scar that forms can make walking uncomfortable. The advantage of the plantar approach is that the neuroma can be reached easily and resected without cutting any structures.

Surgical Complications

The surgical area contains very small blood vessels, nerves, and muscles and complications can occur. Once the neuroma is removed, the empty space may fill with blood, resulting in a painful hematoma. There is a risk for infection, necessitating careful monitoring by the podiatrist and patient. If the incision site becomes warm or red within a day or two after surgery, or if the patient runs a fever, the surgeon must be contacted immediately.

Recurrence is another possibility. The stump of nerve remaining after resection can begin to grow again. If this occurs, the nerve grows in width and length, creating a burning pain that can be treated by injection or further surgery.
